SOLVED: Multiple issues: Crashing to desktop, black screen and glitching during game

Here is a video which I have recorded. I turned the res down and put everything on low settings to see if it was something to do with the system which seems fine to me on other games.
Basically there are about 3 main issues.
1) Crashing to desktop - This can happen when just 1 player comes onto the screen. In a town its usually OK but anything can make it crash. This is usually when moving or even standing still just watching the screen
2) Black screen then crash. Like above but nothing on the screen when logging in and it just crashes (although this has only happened (twice).
3) The worst issue is the glitching during game. You can move from A to B on the screen. Then you will teleport back to A. I have seen multiple people having the same problem. A start and stop happens on the character, they move a little then stop then move a bit more then stop and this happens until you quit the game, then log in again. It may be OK for 10 mins, but it happens all over again until you log out.

I managed to solve this issue. It turns out there was an async route somewhere between my network and ISP UK>USA. This has now resolved the issue.
