I have run in to a bug that happens when you swap to an item on the 1-5 hotbar and immediately use it or a weapon ability it will not function. If there is supposed to be a short global cooldown where an item cant be used immediately after swapping this is fine, however, the particle effects, sound effects, and animations still play as if the ability was used.
The easiest way to duplicate this is to have a piece of armor on your hotbar. Press the number to swap to it and immediately left click to equip it. You will get the armor equip sound effect and your character will go through the short animation, however there is no cast bar and the armor does not get equipped. This will happen for weapon active abilities as well, however it is harder to duplicate.
