Open world dungeons. How will they work?

Since 80% of dungeons will be open world, I have few questions about the general idea:

1. Will there be any systems preventing zerg groups occupying whole area, or running through it instantly. Any difficulty scaling base on number of people inside?
2. Will mobs respawn?
3. Will players be automatically flagged as combatants inside dungeons?
4. What exactly players gets for taking specific spots in dungeon instead just running to the boss area?
5. Will there be single boss at the end or many bosses rooms?
6. Can other group enter boss chamber at the end if other group is fighting boss already?
7. Will dungeon still be open after defeating a boss or will be closed/empty for a specific time, preventing players to run it over and over again.
8. Any random events inside dungeon to make it more diverse and less repeatable?
9. Will there be multiple entry points?
10. Will they be mandatory to get specific items/resources or optional but more profitable?
11. Will they be hard? I wish some of them will stay undefeated for years :)
