Commissions not completing

Description
I took the commission to kill Juicer the bear, killed him with my group and received xp and loot from the kill but did not complete the commission. This has happened for several other kill commissions (corvus, grave goblins) and for explore commissions (ambush island, sightseeing island, and others). Sometimes the people in my party are able to complete them and sometimes we all fail too. It's seemingly random. Some of my commissions have no issues whatsoever.

Bug Reproduction Steps
Sometimes for the explore running across a bridge or exploring a whole small island. For the kill quests killing the specific mob or type of mobs with no results or increase in the count.

Expected Result
Commission completion

Bug Report - Actual Result
The commissions remain outstanding regardless of the number of times I kill the type or specific mob or explore the location.
