Fix mob aggro/leashing/reset

Lower-level players shouldn't be getting ganked & 1-shot by high-level 3-star mobs that got pulled out of position and never leash/reset properly, and now they're just sitting in lower-level areas that should be safe from them. It's especially infuriating when you can't see the mob until they're almost on top of you due to the server lag.
