Constantly Falling Through World

My first two characters would consistently fall through bridges and would fall through the map when crossing large rivers, but now the most recent (third) character I created immediately falls through the world on spawn. When I revive, same deal; immediately falls through the world.
I haven't seen a single other mention of this, so I'm pretty sure my account specifically is an issue. My first characters were all nearly identical in model, so I thought maybe there was a model specific issue; I tried changing the race and size, etc. Nope. It's every single character for me. In fact, the one I made to be larger was worse.
