I like the concept of a “nickname” or “secret identity.”
Like… my character name is Bruce Wayne, but my secret identity is Batman. Ideally there could be game mechanics around that, if you wanted or needed to conceal your identity, this would be a vehicle to do so. Well if someone approached you and called you by a secret name, you would know they were a member of the secret organisation you belong to.
That organisation could be nice or nasty.
Surnames use to relate to role/profession at one time.
So perhaps...name/surname could be organisation specific.

The idea might work like associating a secret identity with a disguise... a costume or outfit that would temporarily hide your identity. I'd have to do some digging, but CoE and Revival both have this concept with really neat use cases. I'll do some research!
