Most games have drowning if you're underwater and burning if you stand in a fire.
What about dehydration in the desert unless you consume water; freezing in the cold regions if not properly dressed; and lip chapping for windy areas?
Not talking about micromanaging hunger / thirst like in a survival game. That would become irritating very quickly. Just general periodic damage ticks if you aren't prepared for regional conditions......

So it will effect our stats to add realism...
...but will it effect our physical appearance ?
...will it effect our animations ?
You are heading into a force 10 gale.....movespeed -50% in this direction....+50% in the other.
You are walking on ice....large chance of slippage.
Maybe in winter seasons Tavern visits will help reduce "chill" debuffs xD who knows xD can't wait to find out xD
Also a city in the desert would be a good place to sell water then, or almost dried out people you find across the road, or just help them
Or vodka, if all else fails.
I think they should add certain elements, but not be too extreme
