Its way easier to code instanced mechanics than it is to code for open world mechanics, as you have a fixed variable to work with. Thus instancing allows you to finely control an encounter's difficulty level. That's why the most challenging PvE content has almost always been instanced.

That would be really cool. But its incredibly difficult to implement that in game, that's why next to no game has this feature.
